Make sure to put your name and address and keep a copy. It is a good idea to make clear you are asking for the information under the Data Protection Act 1998.
So we can be satisfied that you are entitled to receive the personal information you have requested, we will usually require a copy of two forms of identification. One should relate to you personally, for example a copy of a passport or photo driving license; the other should be a recent document showing your address, for example a utility bill.
We also need to know how you would like any information that is found transmitted. The options are:
1. We will send the documents in hard copy by Recorded Delivery.
2. We will send them by e-mail (if the number and size of files is not excessive).
3. We will send them on a CD by Recorded Delivery.
A fee of £10 is payable for Data Protection requests, we will inform you if this applies once we have received your request.
We keep a record of Data Protection requests made, and the responses given, for two years after the date of last contact with the requestor, after which time the request is destroyed.
